Hi, I am kashi Supriya  of 3rd bds student. This is an online elog book to discuss our patients health data after taking his consent. This also reflects my patient centered online learning portfolio.   Case History  A 48 years old male Resident of kurumarthi,work as daily labour presented with the  Chief Complaint of Abdominal pain and Constipation along with abdominal distension/Swelling since one month  History of present  illness  Patient was apparently asymptomatic 1 month back but suddenly had severe Abdominal pain with Abdominal distension/Swelling due to bloating òf gas and constipation  without bowel movement even a single day. Abdominal pain is dull aching type that radiated to backwards and this pain is aggrievated by eating food and relieved on medication...and the patient was unable to lift the weight as that he does in labour work. so,he came to hospital for treatment due to severe Abdominal pain .After using medication here, he passed the stools with flatus.  History of p

Gm case 2  Case scenario..... Hi, this is k.Supriya, IIIrd BDS. This is an online eblog book discuss our patients health data after taking her consent.This also reflects my patient centered online learning portfolio.  CASESHEET:a 85 year old women has come to hospital in an unresponsive state.  Chief complaints: fever and chills since past 10days,burning Micturition and pain in the abdomen, vomitings, and also not taking fuilds or liquids since 2 days.  HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic before 1month. Since 1 month the patient is suffering from mild joint pain. And then the patient was suffering from fever and abdominal pain since 10 days, and loss of appetite I.e. not taking any fluids or liquids from last 2 days. Suddenly the patient was unconscious and was not talking to anyone. Associated diseases: Hypotension  Gastric acidity.   History of past illness: The patient has taken an gall bladder removal surgery(cholecystectomy) before 10 years. PERSONA

This also reflects my patient centerd online learning portfolio..                             CASE   HISTORY Patient details A 37 yrs old male , resident of suryapet nd works as a jr assistant in a social welfare association .. Chief complaints Patient was admitted in the hospital to get rid of alcohol and tobacco addiction .. History of present illness Patient was apparently asymptomatic 16 yrs ago and he's been consuming alcohol and tobacco from 2008 . Patient developed few symptoms after sudden stopage of alcohol ... Are  Fever ( high grade , intermittent ,) with chills from 8 days Vomiting ( thick consistency , watery content) More than 5 times daily since 8 days Loose stool from 5 days..     Aggrevating factors: epigastric pain while standing & walking Relieving factors : relieved on medication History of past illness :  Patient
